We aim to review all our patients who are prescribed medication by the practice, annually, as part of our medication review programme. For some patients this happens as part of a review for their specific medical problems.
Patients who are only prescribed a small number of medications, may not need to have an appointment with the surgery for their medications to be satisfactorily reviewed, if everything is going well and any required monitoring is up to date.
Therefore, we would be grateful if you would complete the following questionnaire and return this to the practice, via the repeat prescriptions box or receptionist, or by email, before your next prescription is due. We will then assess whether any further action is needed for your annual review.
