Home Blood Pressure Readings
If you have been asked by a GP to record your blood pressure or are on medication to control your blood pressure (BP), monitoring over seven days at home is a better way of understanding how well controlled your blood pressure is than isolated readings in the surgery. Please record your blood pressure readings using the practice form or you can pick up a copy from the surgery.
